could either be viral or bacterial , moree commonly viral
viral : mumps
bacterial : strep pneumonia
h influenzae
staph aureus
what are the predisposing factors of sialadenitis ?
Sjogrens syndrome
sialothiasis
Cushings
Hypothyroidism
Diabetes
Hepatic and Renal failure
Some drugs
Immunodeficiency
Anorexia or bulimia
what is the clinical picture of sialadenitis ?
acute onset of pain of thee parotid or submandibular gland that may be associated with:
suppurative discharge from the punctum of the salivary gland
high fever
marked swelling
pain may increase upon mastication
what are the investigations that should be done for a case of suspected sialadenitis ?
Ultrasound
X-ray - to reveal any stones
what is the treatment for bacterial causes of sialadenitis ?
antibiotics
abscess drainage if there is one
analgesics and antipyretics
what is the treatment for viral causes of sialadeenitis ?
no specific treatmeent just conservatiivee
n cases of chronic or recurrent infections what is the best next step in management ?
surgical excision
what is the commonest predisposing factor associated with sialadenitis ?
sialothiasis
what must be excluded in cases of bilateral parotid swelling with fever ?
mumps
by paroxysm virus
what are the complications associated with drainage of parotid abscess ?
fistula formation
