Shock Ch. 10 Flashcards
Signs of compensated shock (6)
- slight tachy, 2. slight delayed cap refill, 3. subtle mental changes, VASOCONSTRICTION leading to 4. slight ^ in DBP, 5. Narrow PP
Signs of early WARM septic shock (3)
- bounding pulse, 2. flash cap refill < 1 sec. , 3. WIDENED PP
Memory aid: “WARM = Wide Open”
Vasocdialtion leads to increased blood lfow, leading to bounding pulse, fast cap refill and wide PP as the dilation decreases the DBP.
Hypotensive decompensated shock signs ? (7)
- ^ tachy, 2. tachypnea 2* acidosis, 3. 3rd spacing of fluid, 6. < mental status, 7. cyanosis/mottling, 8. < cap refill, 9. DIC
neonate hypotension range
<60 mm Hg
Infant hypotension range
<70 mm Hg
Child 1-10 yo hypotension range
<70 + 2X age in years mm Hg
Child >10 yo hypotension range
<90 mm Hg
Irreversible shock signs
multi organ failure
Two causes of a WIDENED PP?
Vasodilation casued by either 1. ^ ICP, or early septic shock
One cause of a NARROW pulse pressure?
Vasoconstriction from hypovolemic shock
What is preload?
The amount of blood returning to the heart.
What is afterload?
The amount of resistance blood leaving the left ventricle must overcome to enter the peripheral circulation.
How can tachycardia effect preload?
By reducing filling times
What BP parameters indicate an increase in SVR?
A narrow pulse pressure and increased DBP
