Found unresponsive client
- call rapid response
- request code cart/defib

Calculating intake
- include ice cream, gelatin, water, juice, cola
- ice chips = half volume
- pureed foods are not included
Giving PPI, avoid
- avoid giving with other meds
- decreases absoption

Obtaining wound culture
- clense with sterile normal saline
- collect using Levine technique
- rotate swab with pressure to express fluid
- do not physically touch wound
- clean gloves OK, not sterile
Breast exam
- sitting or supine
- pillow placed under shoulder of side that is being assessed
- when sitting
- sit erect
- arms at side, or raised overhead
- when flat
- place clients hand on examined side under head
Discontinue oxygen flow
- turn vavle counter clockwise

Braden score
- level of pressure ulcer risk
- high risk is 10-12
- very high is 9 or lower
- 23 means no risk
Side rails restraint
- 3 side rails up is fine
- 4 side rails is restraint
Orthostatic hypotension
- BP cuff over brachial artery
- supine, sitting, standing
- wait 3 minutes between each position change
- watch for falls
- IF SYSTOLIC DROPS BY 20 OR DIASTOLIC DROPS BY 10, POSITIVE FOR ORTHOSTATIC HYPOTENSION
