RX100 Willow Inpatient Fundamentals - Chapter 3: Supporting Clinicians Flashcards
formulary
list of medications selected by the hospital P&T committee, usually on the basis of cost and effectiveness
medication reconciliation
reviewing a patient’s list of current medications and updating it in Epic
physicians can then evaluate continuation or holding of home medications during an inpatient admission
navigator
alternative medication
while reconciling home medications for an admission, the goal is to continue with appropriate, formulary medications that can be ordered/dispensed in the inpatient setting
during this process, the system might display ALTERNATIVES, which guide clinicians to:
- select a more specific medication that includes drug, form, and strength
- select a formulary medication that is therapeutically similar
alternatives can be configured to allow providers to continue with the original order, and provide a reason
during verification, pharmacists can see if an alternative has been accepted, or rejected, and the reason if it was entered
order set
order sets group together orders, medications and non-medications that are commonly needed for a specific purpose, like an admission, to make ordering more efficient
SmartGroup
order sets are organized into sections called SmartGroups
when you open an order set, some SmartGroups are required and others are not
SmartGroups can be expanded or collapsed by default
SmartGroups can also be nested within other SmartGroups, which allows for branching; rather than presenting all the potential orders, the right ones can be shown at the right time
in-line med warning
may pop up while placing orders in SmartGroups or order sets
designed to offer decision support as the provider completes their workflow
non-formulary request
how are patient medications managed upon admission?
how to update patient’s home medication list?
how to identify patient-reported meds vs outpatient prescription meds?
how to order a home med for current hospitalization?
physicians use Patient Lists to look up admitted patients
the provider would then use the admission Navigator to reconcile home meds
what does it mean to “order” a med while reconciling home medications?
what does it mean to “don’t order” a med while reconciling home medications?
what does it mean to “discontinue” a med while reconciling home medications?
