Polycystic ovarian syndrome

oligo/amenorrhoea
hyperandrogenism - clinical/lab findings
polycystic ovaries on USS
Other features of PCOS
Insulin resistance (can lead to increased risk of developing DM)
Impaired glucose tolerance
Insulin lowers levels of SHBG resulting in hyperandrogenism
LH can mimic insulin and there are higher levels of LH than FSH in PCOS
Obesity
Acne
Hirsutism
Acanthosis nigricans
Investigations for PCOS
Hormone profile - oestrogen, TFT, androgens, FSH, LH
Blood glucose
UPT
USS
Management of PCOS
weight loss
DM control - Metformin, diet
COCP - helps to give withdrawal bleed (as progesterone is deficient and helps to prevent hyperplasia as well as increasing SHBG levels which decrease androgen levels)
ovulation induction - clomifene citrate, tamoxifen, letroxole
diathermy
