- Initiation of a T.S. Required shutdown
- ECCS actuation that results or should have resulted in a discharge into the RCS (via a valid actuation signal) *
- Rx trip from criticality *
- Any news release that may generate national attention
* unless trip or ECCS is part of a pre-planned evolution (testing, normal shutdown trip)

What are the conditions that warrant an 8 hour NRC report notification?
- Condition of plant seriously degraded (fission prod barriers)
- plant in unanalyzed condition (that significantly degrades plant “system safety function(s)”)
- valid actuation of following: (unless part of planned evolution)
- RPS
- Cnmt Isolation
- ECCS
- AFW
- Cnmt heat removal/ depressurization
- DG’s
- condition that could have prevented a safety function from being executed had an actuation taken place
- contaminated, injured man transported to offsite facility
- loss of major assessment capability (CR ind, MET tower, etc.)

What are the defined “System Safety Functions” that if lost/degraded requires a report to the NRC?
Who is responsible to make an immediate notification to the NRC if conditions warrent?
- shutdown Rx and maintain in a safe s/d condition
- remove residual heat
- control release of radioactive material
- mitigate the consequences of an accident

What is the “Event discovery date” when it comes to makeing reports to the NRC?
When does it start?
- it is the date and time the clock for reports due to the NRC starts
start times
- date/time tech, engineer, supervisor identifies problem
- if engineering eval is required to determine operability/compliance the date is whenever resonable expectation the equipment operability no longer exists (eval starts to go south)
