Incubation period
20-90 days
Prodromal stage (day 0 - 10)
fever, malaise, headache, paresthesia at bite site
Encephalitic (Furious Rabies)
combativeness, seizures, autonomic dysfunction (hypersalivation, goose flesh, cardiac arrhythmia, priapism)
hydrophobia
Paralytic (Dumb rabies)
early and prominent flaccid muscle weakness
Gold standard of diagnostic of rabies
Fluorescent antibody techniques
WHO Classification Rabies Category I
feeding/touching an animal
licking of intact skin
exposure to px with signs of rabies by sharing or drinking utensils
casual contact w/ patients w/ signs of rabies
routine delivery of health care to a patient w/ manifestations of rabies
Category I treatment
NO vaccine of RIG needed
WHO Classification Category II
nibbling or sucking skin of uncovered skin w/ or w/o bruising
minor scratches/ abrasions w/o bleeding (includes wounds that are induced to bleed)
Category II treatment
start rabies vaccine immediately
WHO Classification Category III
single or multiple transdermal bites or scratches w/ spontaneous bleeding
licks on broken skin or mucous membrane
exposure to rabies patient through bites, contamination of mucous
membrane or open skin lesions w/ body fluids through splattering or mouth-to-mouth resuscitation
handling of infected carcass or ingestion of raw infected meat
suspected contact w/ bats
all category II exposures on the head and neck are considered as category III
Category III treatment
start rabies vaccine and RIG immediately
