Cor Pulmonale
Altered RV structure and/or function in the context of chronic lung disease and is triggered by the onset of pulmonary HPN
Acute RV dilatation and failure occurs d.t. sudden rise in pulmonary pressures (massive pulmonary embolism) but RV does NOT develop hypertrophy
Acute Cor Pulmonale
More slowly evolving and progressive pulmonary HPN –> both RV HYPERTROPHY and DILATION
Chronic Cor Pulmonale
MC symptom of cor pulmonale
dyspnea
Happens d.t. backflow from R sided heart failure
abdominal pain and ascites
Increase in the intensity of the holosystolic murmur of TR w/ INSPIRATION
Carvallo’s sign
A late finding and is secondary to low CO
Cyanosis
ECG findings in Cor Pulmonale
P pulmonale ( p waves > 2.5 mV in leads II and/or VI)
RAD and RV hypertrophy
Chest radiography findings in Cor Pulmonale
enlargement of the main PA, hilar vessels and descending R PA
2D echo findings in Cor Pulmonale
R sided chamber enlargement w/ associated dysfunction; pulmonary HPN
Identifies obstructive, restrictive pulmonary parenchymal disease
Spirometry
Identifies thromboembolic diseases, pulmonary interstitial diseases
