What is peripheral artery disease?
The narrowing/blockage of the vessels that carry blood from the heart to legs
What are different types of PAD?
Acute limb ischaemia
Critical limb ischaemia
Intermittent claudication
What are symptoms of PAD?
Hair loss on your legs and feet
numbness or weakness in legs
brittle, slow growing toe nails
ulcers(open sore) on feet that do not heal
changing skin colour-paler/blue
Leg pain when excercising
How would you manage critical limb ischaemia?
Pain management
Foot care-
daily foot inspection, keeping ischaemic feet clean to avoid infection, being careful to avoid injury when cutting the toenails, avoiding walking barefoot, and wearing well-fitting shoes.
smoking cessation
Vascular MDT
Advise on driving
Diagnosed w/ABPI
How would you manage intermittent claudication?
Supervised exercise programme
Refer for consideration of angioplasty or bypass
consider prescribing naftridrofuryl oxalate
What are main risk factors?
Smoking
DM
What are two types of PAD
Occlusive PAD (e.g intermittent claudication)
Vasoplastic PAD (e.g raynauds syndrome)
What are examples of vasopastic PVD?
How would you treat it?
Raynauds syndrome
Lack of blood flow to extremities.
Avoid smoking and avoid exposure to cold
Nifedipine-prophylaxis
Naftidrofuryl oxalate and prazosin are licensed for Raynauds syndrome
Prazosin
What drugs are licensed for intermitted claudication?
Cilostazol
Pentoxifylline
What can reduced this risk of cardiovascular events in occlusive PAD?
Smoking cessation
effective BP
regulating blood lipids
good glycemic control
anti platelet therapy
diet and weight management and increasing exercise
What is used for the treatment for severe chronic limb ischaemia in patients ar risk of amputation where surgery had failed or is unsuitable?
Iloprost IV
