What are side effects?
hyperkalaemia, which can lead to muscle weakness, arrhythmias, and even cardiac arrest.
Spironolactone causes gynaecomastia (enlargement of male breast tissue), which can have a significant impact on adherence in men (see COMMUNICATION). Eplerenone is less likely to cause endocrine side effects.
Aldosterone antagonists can cause liver impairment and jaundice and spironolactone is a cause of Stevens–Johnson syndrome (a T-cell-mediated hypersensitivity reaction) that causes a bullous skin eruption.
What is the ONLY indication of Eplerenone?
Adjunct in stable patients with reduced ejection fraction or MI
chronic mild HF
What are important interactions of aldosterone antagonist?
Manufacturer advises max. dose 25 mg daily with concurrent use of amiodarone or moderate inhibitors of CYP3A4
what are indications of sprinolactone?
Oedema
Ascites in cirrhosis
Resistant HTN
Hyperaldosteronism in patients awaiting surgery
