What is first line treatment for symptoms relief?
GTN
What counselling points would you give for GTN
If they experience chest pain;
-Stop what your doing and rest
-Take a second dose if the pain has not eased after 5 minutes
-Call 999 for an ambulance if the pain has not eased 5 minutes after second dose or earlier if the pain is intensifying or person is unwell
What is first line regular treatment to reduce symptoms?
BB or CCB
What is second line treatment?
Monotherapy of:
long acting nitrate
nicorandil
ivabradine
ranolazine
What is drug treatment for secondary prevention of angina?
-Antiplatelet treatment in all people with stable angina
- low dose aspirin 75mg daily
What would you give patient with stable angina and diabetes?
ACEi
How would you manage the CV risk in a person with angina? (Practical ways)
-optimise the management of co-morbid conditions
-stop smoking
-cardioprotective diet
-weight managment
-physical activity
- limit alcohol consumption
what symptoms classify typical and atypical angina?
Typical:
->chest pain precipitated by physical exertion
->constricting discomfort on the from of chest, neck, shoulders jaw or arms
- >relieved by rest or GTN
-Atypical:
typical symptoms plus GI discomfort and or breathlessness and/or angina
What factors make a diagnosis of stable angina more likely?
male sex
increasing age
presence of CVD risk factors
history of establised coronary artery disease
What factors that make a diagnosis of stable angina less likely?
-Pain that is continous or prolonged
-pain that is unrelated to activity
-pain that is brought on by breathing
-pain that is associated with dizziness
-palpitations tingling or difficulty swallowing
