aortoilliac PVD (bad)
calf claudication is symptom of
superficial femoral PVD (or tibial)
gangrene/foot ulcer is symptom of
tibial PVD
claudication
effort associate, benign, resolves with rest
rest pain
dull, aching, worse supine, LIMB RISK
exam of PVD
decreased pulse, bruit, muscle atrophy, ulcer/gangrene
diag PVD
functional testing (ABI, pulse volume, duplex ultrasound) and anatomic testing
limb loss- 5 p’s
acute, pulselessness, pallor, pain, paralysis, paresthesia
PVD treatment
behavior change (smoking, diet, activity), medical (BP, HLD, DM), endovascular treatment
diabetes and PVD
sensory delay, non healing foot ulcer, accelerated, decreased inflame response, increased stenosis, use glucose
aortic aneurysm
most infra renal, rate, risk: make, smoke, CAD, FH,
path of AA
increased pressure, weak wall, large radius
direction
spontaneuous, intima, false channel> distal tear. risk for occulusion/rupture
type B AA
decanting. less common/emergent
type A AA
Proximal. 1= entire thing, 2=just arch
