Process Determining Unstable Plaque Formation

Subendocardial vs. Transmural Infarction
Subendo has necrosis limited to inner 1/3 to 1/2 due to reduction in blood flow (usually not complete occlusion)
Transmural is entire wall, usually from complete obstruction
Danger like 3-7 days Post-MI
Have all the inflammatory necrotic stuff going on, like PMNs and macs eating shit and repairing, making tissue soft and possibly rupturing
Ventricular Aneurysm
Late complication from large transmural infarct. Over time, scar gets stretched out and can bulge. Rarely ruptures though
Sudden Cardiac Death
Ultimate mech usually due to lethal arrhythmia triggered by e- instability of ischemic myocardium
