What drug class is levodopa carbadopa?
Dopamine receptors
What is the action of levodopa carbadopa?
Levadopa: leaves/ increases dopamine in brain
Carbadopa: prevents breakdown of levodopa, so it can work longer and stronger
What are the side effects of levodopa carbadopa?
Hallucinations, hypotension (wide QRS,) depression w/suicidal tendencies, tachycardia, angina, ectopic heart beats, palpitations, anxiety, dementia, drowsiness, anorexia, HA, N/V
What is a normal, but alarming side effect of levodopa carbadopa?
Red/brown urine, sweat, and secretions
What are pt teaching with levodopa carbadopa?
No B6 or iron supplements
No high protein diets
Does not cure, this is a lifetime drug
Slow position changes
What do we do because levodopa carbadopa has a slow onset?
Slow onset = slow taper off
Do not stop abruptly = complete loss of movement
What are the contraindications of levodopa carbadopa?
Glaucoma, skin lesions, melanoma, severe cardiac issues, renal/nepatic/ endocrine disorders
What are the signs of toxicity of levodopa carbadopa
Dyskenesia, profusions, involuntary movements (ticks) ) face twitching/ grimacing
What drugs do we not mix with levodopa carbadopa?
MAOI’s = hypertensive crisis
Benzos = CNS depressing
What drug class is talcapone in?
Catechol- d- Methyl tranferase
What is the action of talcapone?
Inhibits metabolism and COMT which prolongs and increases the supply of dopamine in the brain
Allows levodopa to stay in bloodstream
What are the side effects of talcapone?
Liver failure, CNS/derma/ cardiovascular/ GI/ respiratory systems all affected
Dizziness, Psychosis, dry mouth
Orthostatic Hypotension
What is the black box warning for talcapone?
Liver failure
What do we monitor while administering Talcapone??
Ast/alt
What are some patient teachings with talcapone
Slow position changes
Do not stop taking abruptly
This drug does not cure! Only treats symptoms.
