  • Restrict Na <2.4 g/d (helps manage volume)
  • Avoid medications like NSAIDs (increase Na/water retention)
  • Recommend routine immunizations like flu/pneumonia

PAH Diagnosis

A
  • Right-heart catheterization to confirm pressure level is needed (PAP >= 25), must have normal fluid status
  • Vasoreactivity test then done to determine responsiveness to CCB (drops more than 10 mmHg and under 40 mmHg overall)

A
  • Loop diuretic (volume control)
  • Digoxin (improve cardiac output/HR)
  • *-Warfarin (due to prothrombotic state) - goal INR 1.5-2.5 (Note: lower than normal)**

Prostacyclin Analog Information

A
  • Warnings: vasodilation rxn (flushing, hypotension), rebound PH (stopping/dose decrease), bloodstream infections with continuous IV infusions
  • SE: hypotension, flushing, jaw pain, infusion-site pain (IV/SQ)
  • Epoprostenol must be protected from light, even during infusion
  • Flolan specifically must also have ice packs for stability once reconstituted
