NP: Chapter 21 Parkinsons Flashcards
parkinson =
spectrum of progressive neurodegenerative disorders which involve various motor symptoms
ook andere naam voor de symptomen
hypokinetic-rigid syndrome, parkinsonism
hypokinetic-rigid syndrome, 4 symptoms
- lack of movement/slowness
- rigidity
- rest tremor
- postural instability
lack of movement/slowness soorten
akinesia
hypokinesia
bradykinesia
akinesia
when a movement cannot be started immediately
hypokinesia
decreased bodily movement, as manifested by limited facial expression or mask-like facial expression
rigidity
tightness and sore muscles, movements are stiff and jerky
rest tremor
can occur in hands, legs and head, gewoon tremor
postural instability
forward bent posture, can easily fall
non motor symptoms
fatigue
hyposmia (smell disturbed)
sleep disorders
apathy
cognitive impairments
sexual disinhibition
impulse control
hallucinations
hoeveel mensen met PD hebben fatigue en hoeveel hebben pain en hoeveel sleep
fatigue = 50%
pain = 2/3
sleep = 60-98%
how do we diagnose pd
diagnosis is only sure post mortem!
maar established als er bradykinesia is met een van de volgende:
rigidity
rest tremor
postural instability
prevalence pd in nl
2,38 per 1000
parkinsons gender
women more than men
hoehn and yahr scale
- symptoms on one side of the body
- symptoms on both sides, no balance disorders
- symptoms on both sides, with balance disorders, patient can function independently
- both sides, balance, patient requires daily help
- severely disabled, patient is restricted to a char or bed and requires nursing care
parkinsonism in neurons
loss of pigmentation, degenration of dopamine producing neurons in substantia nigra (= compact part)
hoeveel van de dopaminergic neurons need to break down to get parkisonian symptoms
50%
welke medicatie voor pd
levodopa en dopamine agonists
wat zijn bijwerkingen van levodopa
hyperkinetic movemetns or dyskinesia
wat als medicatie niet goed meer werkt
brain surgery, deep brain stimulation
hoeveel % van de mensen met recently diagnosed pd hebben cognitive impairments
24%
welke delen van cogn functions zijn impaired in pd
attention, mental speed, memory, executive functioning, visuospatial deficiencies
hoeveel mensen met pd develop dementia
30%
diagnosis of parkinsons dementia
- diagnosis of pd
- cognitive impairments are progressive
- cognitive impairments interfere with daily functioning
- at least 2 of the four domains: attention, memory, executive functions, visuospatial functions
behavioural disturbances are not necessary for diagnosis, but can occur: hallucinations, delusions, agitation, excessive daytime drowsiness, depression, fear, apathy
