What is observation ?
A
- monitor cource disease
- initiate palliative care if symptomatic
- PSA + DRE q 6-12m
- invasive biopsies NOT performed only if progression
- maintain QOL
= appropriate elderly, frail + limited life expectancy

What is surveillance?
A
- initiate curative treatment - cancer progresses
- appropriate for younger patients with long LE
- dekay morbidity with treatments like surgery, rad / ADT

What is bilateral orchiectomy?
A
- surgical removal testes
- form ADT - reduces circulating androgens to castration levels
- patients find psychologically unacceptable
- not surgical candidates due to age
- preferred in patients - spinal cord compressions / ureteral obstruction

What is radiation therapy?
A
- treat local / locally advanced PCa with curative intent

External beam radiotherapy - EBRT
Brachytherapy

What is external beam radiotherapy? EBRT
A
dose 75-69 GY delivered in 35-41 fractions - low grade PCa
up to 81 Gy for intermediate / high grade PCa
non -invasive
lay on bed with high energy
later stage disease - short EBRT used for palliative care

A
- permanent implantation radioactive beads
- resevered low-risk cancers

A
- in patients resectable disease - surgical removal portion of
- require defintiie therapy based on RF + patient preference

- blood loss
- stricture formation
- incontinence
- lymphocele
- fistula formation
- anaesthesia risk
