Mechanical Ventilation Flashcards
Criteria for weaning
Improvement in primary disease process
Hemodynamically stable
Absence of major organ failure
PEEP < or = 5cmH20
P:F > 150-200 with Fi02 <0.5
Adequate respiratory drive
What is the equation for compliance?
Compliance= delta V divided by delta P
Compliance in a ventilated patient
Tidal volume divided by driving pressure
Driving pressure = Pplat-PEEP
Low compliance equals what
Stiff lungs!
What are the different types of VILI
Barotrauma
Volutrauma
Bio trauma
Actelectotrauma
Common factors that reduce likelihood of successful weaning
Lack of resolution of primary disease
Acquired resp muscle weakness
1. Inadequate recovery from drugs/NMB/sedation
2. Increase work of breathing (increased airway resistance, decreased lung compliance, decreased chest wall compliance)
3. Cardiovascular instability
