Plantarflexion of the ankle (Talocrural joint)
– Patient is prone with knee bent to 90 degrees
- one hand around calcaneus and one hand around front of the ankle
- use calcaneus to move into planterflexion (point toes)
PA GLIDE
(POSTERIOR TO ANTERIOR) GLIDE
AP GLIDE
(ANTERIOR TO POSTERIOR) GLIDE
Ankle PA
–Patient is prone with knee bent to 90 degrees
–Fingers around calcaneus and one hand around ankle
- Push against each other
Dorsiflexion of ankle
– Patient is supine and leg is straight
- One hand around the calcaneus with the sole of the foot on our forearm
- Other hand holding just below the knee for stability
- Pull up for dorsiflexion
Ankle AP
– Patient is prone and knee is bent 90 degrees
– One hand around front of ankle/talus (ankle must be in PlantarFlexion )
- Other hand just below
- Push against each other
Dorsiflexion/AP MWM
– In standing ask patient to put affected foot onto a step/stool
–Using thumbs locate talus
–Maintain pressure over talus, using pads of thumbs, ask patient to lunge forward
Knee Extension
–Patient is in supine
- Put knee into flexion and then stretch out into extension
- Put one arm on the ankle and the other just above the knee
Knee Flexion
–Patient is in supine
–Passively test their flexion ROM
–Support around the knee and around the foot and perform all grades in this position
- If knee is very sore could do with patient prone
Antero-posterior and Postero-anterior Mob (Knee)
–Support knee with a pillow or fix in 70-80 degrees flexion
–Grasp around proximal tibia and perform technique
Medial glides of patella
–Patient is supine with knee in extension
–Place the pads of the thumbs against the lateral border of the patella and glide patella medially
Lateral glides of patella
–Patient is supine with knee in extension
–Place the pads of the thumbs against the lateral border of the patella and glide patella laterally
Longitudinal caudad and cephalad (knee)
–Start with patient in supine and knee in extension
–With one hand cup the inferior aspect of the patella. The heel of the other hand is placed of the superior aspect of the patella.
–From this position you can glide the patella in a caudad or cephalad position to ascertain ROM and then perform the specific mobilisation
1ST MTP joint (big toe) Extension and flexion
- Lay patient supine with foot off the bed
- Pinch the end of the big toe with the heel of foot
- Hold the big toe and lift up to extend
- For flexion push toe down
1ST MTP joint (big toe) distraction
- Pinch the end of the big toe with the heel of foot
- Pull the big toe towards you
Metatarsal joints
- lay patient supine with feet hanging off bed
- Hold the ball of the foot and the front of the foot
- Move in coordination to extension or flexion inversion or eversion
Superior tibiofibula joint
- Patient sitting on bed with knee flexed at 70 degrees
- Sit on the foot of the patient
- Hands cupping the back of the knee and perform a glide with one hand stabilising
Ankle inversion and eversion
- Put patient on bed in supine
- Cup the heel with one hand
- And other hand on the front of the foot
- Use the heel to move left and right into inversion and eversion
