Who is the MAIB?
Marine Accident Investigation Bureau - investigates marine accidents involving UK vessels worldwide and all vessels in UK territorial waters.
They are an independant unit within the DoT. Their job is to prevent futher avoidable accidents from occuing, not establishing blame or liability. Based in Southamption
Formed out of the Herald of Free Enterprise accident in 1987

MAIB responsibilites
-Carry out investigations and determine the causes of accidents
-Increase the awareness of how marine accidents happen
-Publish reports that include recommendations
Under SOLAS each Flag State is required to investigate casualties occuring at sea.
Master responsibilities
The Master on a UK flagged vessel must report an incident or casualty to the MAIB
The master on any flagged vessel must report an incident or casualty which occurs in UK waters to the MAIB.
Very serious marine casualty is total loss of ship, severe damage to marine enviornment or death
Marine casualty is:
-Death or serious injury (hospitalisation / incapacitation of more than 72 hours)
-Loss of person from a ship
-Loss, abandonment of a ship
-Material damage to a ship (structural integrity / performance damage to ship)
Marine incident:
-Close quarters situation where urgent action was required to avoid collision
-Any event which had the potential to result in serious injury
-Fire not resulting in material damage
-Unintended grounding (no risk of material damage)
-A person overboard wich was recovered
Could call them or fill in appropriate form (ARF)
