All passenger vessels and all other vessel 500GT and over. Created after the 1987 Herald of Free Enterprise accident where the bow door was left open on a Ro-Ro ferry. And Scandanavia Star in 1990
The ISM code sets an international standard for the safe management and operations of ships through the implementation of a clearly defined, structured and documented safety management system.
80% of shipping accidents are caused by human error, and an underlying human act or omission plays some part in virtually all accidents including those where structural or equipment failure seem to be the immediate cause.
ISM Code requirements include:
Having a structured and documented SMS that is followed by ships and company’s personnel.

What are the contents of the ISM Manual
“RP-CEERPS”
- Roles and Responsibilities: To ensure no task is left undone
- Procedures: For all known or expected procedures; bridge, deck, ER, interior etc
- Crew Familiarisation: Designated to Safety Officer by Master who will ensure it is completed in a timely manner 24hrs - LSA, Escape Routes, Must points etc
- Emergency Procedures: Man Overboard, Fire, Grounding, Collision, Separate file on the bridge, laminated cards, all bridge watch keepers to have read and signed
- Emergency Drills and Training: MGN71 = SOLAS Reg for drills is every month - Also have a drill matrix
- Risk Assessments and Permits to Work
- Planned Maintenance: LSA / FFA (reference to SOLAS, manufactures instructions and MCA guidance) Ships equipment such as lifting equipment amd critica; equipment
- SOPEP: Procedures, Plans, Equipment, Contacts
(1 to 5 = People / 7 = Property / 8 = Environment, Pollution)

What is the role of the DPA?
- Point of contact between ship-shore
- All crew have access to contact details
- Direct contact for emergencies
- Keeps the vessel legal at all time keeping the vessel UTD with new regulations
- Has access to highest levels of management

As master it is my role to:
I - Implement SMS & Safety and Environmental protection policy
M - Motivate crew in the observance of the policy
O - Orders - issues appropriate orders in clear and simple manner
V - Verify that specific requirements are observed
R - Review - (With Company / DPA) the SMS and report its deficiencies
R - Report back to the DPA

ISM - What is the role of the Safety Officer?
-They are the safety adviser aboard a ship.
- Ensure a safe working culture - Carry out safety meetings - Investigate Near Misses, Incidents and Accidents - Risk Assessments - Workplace Inspections (Every 3 months)
Their appointment must be logged in the OLB

Requirements: Copy of DoC must be kept onboard each vessel under company’s mgmt and SMS Records Required for Audits
SMC is invalid immediately if DoC withdrawn (vessel unable to trade)
Interim DoC: Issued to new companies or those wishing to add vessel types to an existing DoC (must have satisfactory SMS and plan to implement it onboard to achieve ISM compliance within interim period)

Sole lookout
Under what code and what situations can you have a sole lookout?
- Under the STCW Code, the OOW may be the sole lookout in daylight provided that on each such occasion
- Assessed: The situation has been carefully assessed and it has been established without doubt that it is safe to operate with a sole lookout;

Full accout has been taken of all relevant factors, including, but not limited to:
- state of weather
- visibility
- traffic density
- proximity of danger to navigation
- the attention necessary when navigating in or near traffic separtation schemes;
- assistance is immediately available to the bridge when any change in the sitation so requires.
If sole lookout watchkeeping practices are to be followed, clear guidance on how they should operate will need to be given in the shipboard operational procedures manual.

Gives guidance on how Merchant Ships can comply with the UK Health and Safety at Work regulations - SI2962 / MGN636. It provides guidance on how statutory obligations can be fulfilled.
It is the Master’s responsibility to ensure the Health and Safety policies are implemented and clearly communicated to all those working onboard. Set the tone for a safety culture onboard.

Where does it state the master has overriding Authority?
The Master has overriding authority as per:

It is stated in the SMS as per the ISM Code:
* Nothing in these instructions removes from the master his authority to take any steps and issue any orders, whether or not they are in accordance with the instructions (SMS) which he consideres are neccessary for the preservation of life, the safety of the ship or the prevention of pollution.* - Ships Security Plan as per the ISPS Code: The company shall ensure the ship security plan contains a clear statement emphasizing the master’s authority. The company shall establish in the ships security plan that the master has the overriding authority and responsibility to make decisions with respect to the safey and security of the ship and to request the assistance of the Company or of any Contracting Government as may be necessary.
Observation / NC / MNC
Observation - means a statement of fact during a safety management audit and substantiated by objective evidence
Non-Conformity means an observed situatuon where objective evidence indicates non-fulfilment of a specified requirement
Major Non-Conformity means an identifiable deviation that poses a serious threat to the safety of personnel or the ship and/or serious risk to the environment that requires immediate corrective actions.

Safety Representatives and Safety Meetings
Powers vs Duties
On every ship with five or more seafarers the Company must make arrangements for the election of safety representatives. Appointment should be noted in the OLB. Seafarers at all levels and in all departments should have effective representation
Safety representative should be at least 18 years of age, no fewer than 2 years consecutive sea service.
Minimum Safety meeting: Chairman (Master), Safety Officer, Safety Representative(s)
Safety meetings should be at least between 4-6 weeks however a meeting should also be held after any serious incident or accident or if a large change in status is about to occur (prior to entering a shipyard period).

Management of change is a set of best practices used when a ship or company makes changes which could expose or create hazards. Its purpose is to successfully roll out a change and train crew in the implementation of the change in order to be successful.
