Lichen Planus Flashcards
def
hronic inflammatory skin condition affecting skin and/or mucosal surfaces
what charac LP
4 p’s
purple polygonal pruritic papules that form plaques
mechh
T cell-mediated autoimmune disease, in which inflammatory cells attack an unknown protein within skin and mucosal keratinocytes
cutaneous
front of the wrists, lower back, and ankles
plaques often have a purple or violet hue, except on palms and soles where they are yellowish brown.
plaques are crossed by fine white lines called Wickham striae
cause of Wickham striae
stretching of skin
Oral lichen planus
inside of the cheeks sides of the tongue, gums and lips
hard palate
sign of more recalcitrant form
Vulval lichen planus
abia majora, labia minora and vaginal introitus.
Scarring, resulting in adhesions
Painful desquamative vaginitis, preventing intercourse, and causing a mucky discharge. The eroded vagina may bleed easily on contac
Penile lichen planus
classical papules in a ring around the glans
Lichen planopilaris
tiny red spiny follicular papules on the scalp
Destruction of the hair follicles leads to permanently bald patches characterised by sparse “lonely hairs”
Nail lichen planus
called twenty-nail dystrophy if all nails are abnormal and nowhere else is affected
sheding of nais
loss of nails (anonychia)
scarring of cuticle
pterygium)
Lichen planus pigmentosus
ill-defined oval, greyish brown marks on the face and neck or trunk and limbs without an inflammatory phase
provoked by sun exposure
causes dark patchy pig of lips
Lichen planus pigmentosus is similar to
Erythema dyschromicum perstans
benign skin condition characterised by well-circumscribed round to oval or irregular patches on the face, neck and trunk that are grey in colour. It is a form of acquired dermal macular hyperpigmentation.
Lichenoid drug eruption
lichen planus-like rash caused by medications.
gold
captopril
hydroxychloroquinone
complications
SCC
-enlarging nodule or an ulcer with thickened edges.
dg
clinical ft’s
patch test for mercury allergy w/ oral LP of gums
skin biopsy
- melanin beneath dermis
- liq degen of epidermal basal layer
- Band of inflammatory cells just beneath the epidermis
