Lab 6 - Secretion Clearance Flashcards
What are 3 key points for postural drainage
What are contraindications for postural drainage
What are contraindications for tipping an individuals head-down
Should we tip someones head down or used modified positions for postural drainage
ONLY USE MODIFIED POSITIONS
Postural drainage positions
What are key points for percussion
What are contraindications to percussion
Contraindication: pulmonary embolus (don’t want to dislodge), heart arrythmias, unconscious, anything that makes them medically unstable (bleeding disorders, tumours -> could cause blood flow to area and feed tumour development)
How to perform percussion
Need a hollow cupping sound
-Want to make sure they lift shirt where you are cupping to ensure there is no redness or signs of infection (so you know redness is caused by you cupping them)
-Can do for like a minute or 2 but can be upwards of 10 minutes (depending on patient and amount of secretion)
-Can do with one hand, 2 hands, fast or slow
-Want patient to cough up secretions
-Can do this on inspiration or expiration (does not matter) -> during normal tidal volume breathing
-Can do over ribs but not over shoulder blade, large area of breast tissue, clavicle, scapula, organs
Key points of vibrations
-Only works on places where rib cage is mobile (anterior, posterior, lateral lower lobe, and maybe the lingula lobes)
-Only do vibrations on expiration
-In kids: percussion would be better than vibration may only use fingers or one hand for vibrations) -> wont be able to do forced inspiration and expiration
-Don’t tip infants or children
*be careful to watch stats and patients face before and after as mucous plugs can shift and block airways
Following percussions and vibrations what should you do
Secretion clearance so coughing or huffing
Can you combine percussion and vibration
Yes, you would do percussion on inspiration and vibration on expiration
