History of unexplained, vaginal bleeding
Pregnancy
History of breast cancer,
Coronary heart disease
Previous Venus thromboembolic, events, or stroke
Active liver disease, 
CI for buproprion use
Seizure disorder
Eating disorder
Alcohol withdrawal
CI triptans
Pregnancy
CAD
Stroke CVD
Ergot use
CI progestin only OCP
Current breast cancer
Liver disease is relative CI
Indication for immediate AOM abx therapy
If <6 month of ago
Bulging TM
Febrile >39C
Systemically ill
Severe otalgia
No improvement in 48hr
Indication for ENT referral for AOM
Recurrent 3x/6 month or 4x/year with middle ear effusion at time of evaluation
Chronic AOM>3 months with risk of speech/learning/hearing
Bilateral OME with hearing loss
OME with vestibular symptoms, discomfort, behavioural problems
Resistant to second line abx
Perforated and not resolved in 6 weeks
CI fibrinolysis
ICH history
Ischaemic stroke within last three months
Presence of cerebral vascular malformation, or primary metastatic intracranial malignancy
Symptoms of aortic dissection
Bleeding, diastasis, or active bleeding with the exception of menses
Significant close head or facial trauma within three months 
CI live vaccines
Pregnant or immune compromise people
Active TV
Analgesics CI in CKD
NSAIDs, tramadol, codeine, morphine
Relative CI antiepileptic in pregnancy
Valproic acid and phenytoin are teratogenic
Minimum effective force and high dose folic acid
CI PDE5i
CHF, hypotension, unstable angina, recent stroke and MI, nitrates
Contraindications to pro kinetics
Full bowel obstruction
Indication for O2 in COPD
PaO2<55mmhg
PaO2<60 with bilateral ankle swelling or cor pulmonale or hematocrit>55%
Indication for US KUB in child with UTI
Recurrent UTI
<2yo with first febrile UTI
Pyelonephritid
Family or personal history of urology or renal abnormalities
