Epinephrine for ACLS adults cardiac arrest
1mg q3-5min IV/IO
Amiodarone in cardiac arrest
300mg bolus first dose
150mg second dose
IV/IO
Lidocaine in cardiac arrest
1-1.5mg/kg first dose
0.5-0.75mg/kg second dose
Adenosine in tachycardia
First dose: 6 mg rapid IV push then NS
Second dose 12mg
Wide QTS tachycardia meds
Procainamide IV: 20-50mg/min until arrhythmia suppressed or max dose 17mg/kg. Maintenance infusion 1-4mg/min
Amiodarone IV: 150mg over 10min followed by maintenance infusion 1mg/min for 6 hours
Sotalol IV: 100mg over 5min unless long QT
Bradycardia meds
Atropine IV: 1mg bolus q3-5min up to 3mg
Dopamine IV: 5-20mcg/kg/min
Epinephrine IV: 2-10mcg/min
Dextrose in hypoglycemia
50ml D50W or 1mg IM glucagon
Naloxone dose
2mg initially up to 10mg if life threatening
otherwise 0.1mg initially doubled q2min up to 10mg
Thiamine for WE
100mg with 25mg dextrose (50ml D50)
Lorazepam in status epilepticus
0.1mg/kg IV only (not IM)
Usually 8mg for adult
Epinephrine dose for pediatric cardiac arrest
0.01mg/kg IV/IO
Toxic dose of acetaminophen
150mg/kg
Midazolam dose in status epilepticus
0.1mg/kg IM/IV/IN
10mg for adult usually
MgSO4 in eclampsia
4g IV during labor (ppx too)
