Rule 11.0
Incident Procedures
A near miss can involve:
-Employee, someone outside the company, property, truck
A driver or helper must report all incidents and near misses immediately to the District
Rule 11.1
Protecting the Scene
If our vehicle is involved in an incident, reflector triangles must be set out at the scene according to federal, provincial, or local regulations
A driver/helper can not leave the scene unless they are given clearance by management or emergency/medical personnel.
Rule 11.2
Statements to other Personnel
While at the scene of an incident, there are some basic rules that a Driver/Helper must follow:
- Do not admit fault or accept liability for the incident
- Do not argue about responsibility for the incident
- Do not make statements until Company representative is present, if possible.
- -This includes talking with police officers and others involved in the accident
Rule 11.3
Alcohol and Drug Test
If a driving incident occurs, WM requires:
- Drivers must make themselves available to Management for at least 8 hours after an incident
- Drivers must not drink alcohol for at least 8 hours after driving, as this could influence post-incident testing
Rule 11.4.1.
Report to Dispatch
Regardless of the situation, a Driver must immediately report an incident to Dispatch
Rule 11.5
Fleet Pride Equipment Incident Program
A Driver and Helper must immediately report any damage to their truck or equipment to dispatch, route Manager or Maintenance
Rule 11.4.2
Complete Incident Form
A driver must complete a vehicle incident report form if they are involved with an incident regarding minor property damage or an unattended vehicle.
-Write down the time and the customer or the public address and location
-Write don the car’s license tag number, make, model, color, and year.
Drivers and Helpers must not withhold information or fail to give all the facts to WM-authorized personnel regarding unusual events, property damage, personal injuries, or operations and safety rules book violations
Rule 11.4.3
Do not Leave the Scene
In the case of minor property damage or an unattended vehicle, a driver can not leave the scene of the incident unless they are instructed by management
Rule 11.6.1
Reporting
Drivers and helpers must immediately report any near misses, incidents, and/or injuries to dispatch and/or the route manager
These reported near misses, incidents, and/or injuries may involve the employee or the public in the following areas:
-On WM property
-AS a result of WM equipment
-As a result of a WM employee’s actions
Rule 11.6.2
Employee Work-Related Injuries
Drivers and helpers must immediately report all injuries to dispatch and/or a route manager
-They should seek medical attention as they deem appropriate or as directed to by their Route Manager
Rule 11.6.3
Employee Non-Work-Related Injuries
A driver and helper must report any injury that could in any way affect their job-related duties to their manager.
To return to work, they must:
-Complete the appropriate forms
-Obtain a full release by a physician to return to work
