hyperthyroidism & hypothyroidism Flashcards
s/s:
- increased metabolic rate
- nervous irritable, palpitations, tachycardia, heat intolerance, excessive perspiration, exophthalmos, increased appetite, wt loss, amenorrhea
hyperthyroidism
s/s:
- slow metabolic rate
- lethargy, constipation, wt gain, complains of being cold even in warm environments, menstrual disturbances, decreased libido
- abnormally sensitive to sedatives, opioids, anesthetic agents
hypothyroidism
- inhibits production thyroid hormones, doesn’t interfere w/ previously stored hormones, might not take effect for a few weeks
- short half life –> give q8h
- BBW: AST/ALT
- pt may require higher dose at first
- interacts w/ anticoagulants
- teach bleeding precautions
- avoid iodine
- AE: dermatologic, dizziness, drowsiness, headache
propylthiouracil
propylthiouracil is used to treat
hyperthyroidism
- beta-blocker
- slows HR, lowers BP
- doesn’t control hormones, helps w/ symptoms
- useful for controlling symptoms during delayed response to other drugs
- don’t give if HR <60 or diastolic <90
- discontinued when pt becomes euthyroid
propanolol
- large amount radiation in a single dose to pt
- must get to an almost normal thyroid level before this
- reduces size of thyroid or destroys diseased thyroid tissue
- teach pt to abide by radiation precautions after tx
radioactive iodine
T/F: after receiving radioactive iodine treatment, the patient may take public transportation, share utensils, and flush regularly
F
- not used for obesity/wt loss purposes
- increases metabolic rate
- hold if Hr >100
- increases HR
levothyroxine sodium
levothyroxine sodium is given to treat
hypothyroidism
