Hormonal agents 2 Flashcards
Androgen Biosynthesis inhibitors include
abiraterone
Abiraterone MoA
inhibitor of the cytochrome P450 enzyme (17 alpha hydroxylase, CYP17A1) - this enzyme is responsible for the biosythesis of estrogen
Abiraterone toxicity
joint swelling, edema, hot flashes, muscle discomfort
What is abiraterone used for?
used for androgen deprivation therapy - usually in ben with late stage castration-resistant prostate cancer
Continuous administration of abiraterone leads to
increase in ACTH levels - mineralcorticoid excess
Abiraterone blocks androgen production from what 3 sources?
testicular, adrenal, prostate tumor tissue
GnRH agonists include
Leuprolide and Goserelin
GnRH agonists MoA
stimulate a release of FSH and LH, then inhibit their release, suppressing testosterone production in the testes
GnRH agonist side effects
hot flashes, impotence, gynecomastia, risk of diabetes and heart disease
Castration levels of testosteronce can be achieved with the use of Leuprolide and Goserelin in how long?
2-4 weeks of treatment
The initial surge of LH and FSH caused by GnRH agonists can lead to
a flare of disease - this can be avoided with androgen receptor blocks
GnRH antagonists include
Degarelix
GnRH MoA
bind reversible to the GnRH receptor in the anterior pituitary, inhibits LH and FSH release
Side effects GnRH antagonists
hot flashes, weight gain, increase in hepatic transaminases, QT prolongation (long term use)
Delgarelix is a
synthetic decapeptide GnRH receptor antagonist used for rapid medical castration - no surge in LH or FSH before inhibition
