Hirschsprung disease
Absence of ganglion cells which causes an absence of peristalsis , and loss of rectosphincteric reflex
What can the absence of ganglion cells cause?
Stool accumulation leading to mega colon leading to intestinal ischemia and entercolitis
Entercolitis
Damage to mucosal cell lining valve wall
Will decrease blood supply and lead to cell death
What diagnoses hirschsprung diasease
X-ray is going to assist in diagnosis but true confirmation is through rectal biopsy
What are they looking for in rectal biopsy
Absence of ganglion cells
What does hirschsprung disease get mis diagnosed for?
Chronic constipation and is initially treated for various forms of constipation
Clinical manifestations of Hirschsprung disease in an infant
Failure to pass meconium
Abdominal distention
(From not passing stool)
Feeding intolerance / vomitting
If an older child gets past infancy what are the manifestations?
Constipation
Diarrhea
And or ribbon like could smelling stool
Easily palpable stool mass
Surgery for Hirschsprung
Endorectal pull through
When they basically cut out the not good part of colon and pull down the healthy one and connect that one
