what does heparin do?
potentiates anti-thrombin = increases inactivation of thrombin
what investigations are useful for LMWH?
anti-Xa assay (5a)
what investigations are useful for unfractionated heparin?
APTT
what are the complications of heparin?
bleeding
heparin induced thrombocytopenia
osteoporosis
what does warfarin do?
inhibits vitamin K = targets clotting factors 10, 9, 7, 2
why should we be careful when initiating warfarin?
initially protein C and S drop = increased risk of VTE occurring
what should we do when initially starting warfarin to prevent VTE risk increasing?
bridge with LMWH
after 4-7 days should be fine
what is the therapeutic window of warfarin?
narrow
how do we monitor warfarin?
INR = patients PTT/mean normal PTT
what is the target INR for patients on warfarin?
2-3
what does the higher the number of INR mean?
less likely of thrombosis occurring, more likely to bleed
what are complications of warfarin?
bleeding
interactions with drugs metabolised in the liver
