What is Guillain-Barre?
Immune mediates demyelination of peripheral nervous system
What is the main trigger of Guillain-Barre?
Campylobacter Jejuni
What are 2 other triggers?
Cytomegalovirus
Epstein-Barr virus
What do B cells create antibodies against initially?
The pathogen
What do these antibodies also match?
The proteins on the nerve cells
What happens to the peripheral nerve cells
Demyelination
What is the main presentation of Guillain Barre?
Progressive, symmetrical, ascending weakness of all limbs- starting at the feet
What are the other symptoms of Gullian Barre?
Reduced reflexes
Neuropathic pain
Proximal muscles affected
What proximal muscles are affected?
Respiratory
Cranial nerve- Facial weakness
How long do symptoms take to develop after infetion?
4 weeks
When do symptoms peak after infection?
2-4 weeks
What is used in diagnosis?
Nerve conduction studies
Lumbar Puncture
What will you see with nerve conduction studies?
Nerve conduction is slowed
What will you see with lumbar puncture?
High protein
Normal white cell count
What is the main management of Guillain-Barre?
IV immunoglobulins
