Name 2 medications that exacerbate GORD
Alpha-blockers and anti-cholinergics
What type of drug is used to treat GORD?
PPI
How is H.pylori treated in gastritis?
PPP and 2 antibiotics (amoxicillin and clarithomycin/metronidazole)
How is peptic ulcer disease managed?
Stop NSAIDS, PPI (H2 receptor antagonist if unresponsive to PPI)
What do prostaglandins do in the GI system?
Potent vasodilators, decrease acid secretion, stimulate bicarb and mucus secretion in stomach
Describe resting vs stimulated phase of a parietal cell
Resting = Proton pumps are in tubovesicles Stimulated = tubovesicles fuse with canalicular membrane where H+ is exchanged for K+
How do PPIs work?
Bind covalently to gastric H/K ATPase irreversibly and block function
Which enzyme metabolises PPIs?
Cytochrom p450
How do H2 receptor antagonists work?
competatively and reversibly inhibit binding of histamine to H2 receptors
What are prostaglandins used for ?
treatment for NSAID induced ulcers
What type of drug is used to treat dyspepsia?
Antacids
What side effect can aluminium hydroxide cause?
Constipation
What side effect can magnesium hydroxide cause?
Diarrhoea
What is a successful treatment for H.pylori?
PPI and 2 antibiotics
