Pathogenesis of extensor mechanism injuries?
- Rapid contractile force: heavy weight lifting
- Spontaneous fall in degenerate tendon
- Patellar tendon rupture: under 40s
- Quadriceps tendon rupture
- Partial tears: some extensor mechanism function but reduced power
Who usually gets quadriceps tendon ruptures?
Over 40s
Who usually gets patellar tendon rupture?
Under 40s
Predisposing factors of extensor mechanism injuries?
- History of tendonitis
- Chronic steroid use
- Diabetes
- RA
- Chronic renal failure
What drugs can cause tendonitis?
-Quinolone antibiotics (ciprofloxacin)
Signs/symptoms of extensor mechanism injuries?
- Straight leg raise test (SLRT)
- Might have palpable gap in extensor mechanism
What should you not treat tendonitis of extensor mechanisms with?
Steroid injections
-Duet o high risk of tendon rupture
Investigation of extensor mechanism injuries?
-X-ray: if there is a palpable gap in extensor mechanism
High patella in x-ray?
Patellar tendon rupture
Low patella on x-ray?
Quadriceps tendon rupture
Treatment of extensor mechanism injury?
- US confirmation
- Tendon to tendon repair
- Reattachment of tendon to patella
Complete/partial tears treament?
Surgical
