Hr Normal, BP Normal, No CCP, Well Perfused, CRT <2, No Oedema, No Palpitations, No Dizziness, No Postural Drop, No LOC, No Blood Loss, ECG - Nil Acute Seen, No Radial-to-Radial Delay. Warm Peripheries.
EPCR CNS
FAST - VE, No LOC, No Head Injury, No Confusion, No Seizure Activity, MSC x 4, No Altered Sensations, No Visual Disturbances, Orientated to Time Date Day & Location, Normal Gait, No Balance Problems, Normal Co-ordination, Pupils PEARL.
EPCR GI
Abdo SNT, No Guarding, No Masses, BS Present all fields, No Nausea, No Vomiting, No Diarrhoea, Normal Appetite, Urine - Reported Normal, Bowels - Reported Normal, No Distention, No Bruising, No Direct Trauma, No Rebound Tenderness, No AAA Felt, No Chance of Pregnancy, LMP =
EPCR CNS
FAST - VE, No LOC, No Head Injury, No Confusion, No Seizure Activity, MSC x 4, No Altered Sensations, No Visual Disturbances, Orientated to Time Date Day & Location, Normal Gait, No Balance Problems, Normal Co-ordination, Pupils PEARL.
EPCR Advice
Call 999 in life threatening emergency. For example if you develop severe shortness of breath and are unable to speak between breaths, if you develop severe chest pain which is new or worse than normal or if you develop sudden one sided paralysis. Call 111/OOH/GP with any questions or queries. Maintain good oral intake.
EPCR GU
Urine reported normal. No pain when passing, non offensive smelling, no blood visible, normal colour. No increase or decrease in urine output.
EPCR MSK
No Head Injury, No Neck Pain, No Back Pain, No Gross Deformity, MSC x 4, Normal Sensations, Normal Mobility, No Crepitus, No Swelling, No Bruising, Normal Circulation distal to injury.
