Pus in the pleural space
Empyema is a common complication of?
Pneumonia (43% of pneumonia patients develop it)
Which type of bacteria is it most commonly associated with?
Anaerobic bacteria
Which gram positive aerobes?
Strep milleri Staph aureus (post-op)
Which gram negative aerobes?
E. coli
Pseudomonas
H. influenza
Klebsiella
What are clinical signs of empyema?
Slow to resolve pneumonia
Persisting effusion on CXR - D-sign
What investigations?
CT
USS
CXR
What treatment?
Chest drain
IV or oral antibiotics
Which IV antibiotics would you give?
Broad spectrum
Amoxicillin and metronidazole initially
Which oral antibiotic would you give?
Directed words the cultured bacteria
What predisposes to chronic pulmonary infections?
Immunodeficiency (antibody deficiencies / hypo-splenism / immune paresis) Immunosuppression Defective Innate Host defence Repeated Insult (recurrent aspiration / indwelling material)
