Q

Q. Ptn presented with sever hyponatremia ( < 109 ) with seizure. What’s the appropriate management?

A

If the ptn was asymptotic he would’ve been treated with fluid restriction. But in this case he’s symptomatic then IV 3% saline at 100ml/ hour should be given ( the correction shouldn’t be more than 2meq/L/h in the first 2 hours for ptn with seizure. the total rise shouldn’t exceed 10meq/L/ 24hours, to avoid the risk for osmotic demyelination syndrome
