ability of EBV TO MAINtain persistent infection is aided by the expression of the
EBNA1 ( ebv nuclear antigen 1) Viral gene product. wc prevente cytotoxic T lymphocytes response to the virus.

IM- Infectious mononucleosis
fever upto 40C
headache
lymphadenopathy
splenomegaly
pharyngitis(sore throat)
cutaneous and mucous membrane lesion- 10% of IM pxs.
edema or eyelids and macular morbilliform eruptipn- most common.
trunk and upper extremity- Eruption usually.
painful genital ulceration may predede symptomatic phase of IM. spec premenarcheal girls.
may be the cause of non sexually relsted genital ulcers
Lipschutz ulcers
lab
absolute lymphocytosis- greater than 50% and monocytosis w abn large “atypical” lymphocytes.
WBC from 10-40,000cells/mm3.
Liver function test may be elevated.
heterophile antibodies- 95% of cases.
IgM and VCA viral Capsid antigen- higher titer and decrease during recovery.
Tx of IM.
No tx.
acyclovir is not effective in alt length or severity of IM.
w severe pharyngeal inv w enroaxhment on the airway; or cytopenias or liver failure- Oral steroid tx (40-60mg/day) of prednisone.
Almost all pxs w IM tx w Ampicillin, amox, or other semisynthetic penicillin typ develop
a generalized pruritic erythematous to copper colored macular exanthem on the 7-10th day of tx.
