Mechanism of Action
Dilates renal vasculature (low dose)
Increases HR, force of contraction, and speed of impulses (mid dose)
Constricts peripheral vasculature (high dose)
Indications
Cardiogenic shock, septic shock, distributive shock, symptomatic bradycardia
Contraindications
Hypovolemic shock, pheochromocytoma (adrenal tumor with increased catecholamine release), tachydysrhythmias, V-fib
Adverse Effects
H/A, anxiety, dyspnea, dysrhythmias, HTN, palpitations, CP, increased myocardial O2 demand, PVC’s, N/V
Precautions
Incompatible with alkaline solutions, MAOI’s and Bretylium will enhance effects, beta blockers will antagonize effects. Do not give with Phenytoin (hypotension, bradycardia, seizures)
Dose and Adminstration
2-20mg/kg/min titrate to effect
2-5mcg/kg/min (low dose)
5-15mcg/kg/min (mid dose)
15-20mcg/kg/min (high dose)
