general description of difficult urination (Long BI)
Long Bi refers to the disease chiefly characterized by difficult urination, oliguria or dribbling urination, or anuria in severe cases. Long is the term for a slow onset of the disease with dysuria and scanty dribbling urination; Bi is the term for an acute attack of the disease with anuria.
differentiations for difficult urination
- Damp heat in the bladder
- excessive accumulation of lung heat
- stagnation of the liver qi
- urethral obstruction
- insufficiency of spleen qi
- Declining of kidney yang
damp heat in the bladder
a. Manifestations: Urine retention of oliguria, scanty dark yellow urination with burning sensation; red tongue and yellow greasy coating, soft and rapid pulse.
b. Treatment principle: Eliminate damp heat and promote urination
c. Acupoint selection: REN3, BL28, BL39, SP9, SP6
Excessive accumulation of lung heat
a. Manifestations: Urine retention or dribbling urination, red tongue and thin yellow coating, rapid pulse
b. Treatment Principle: Clear away heat in the lung to promote urination.
c. Point Selection: REN3, BL28, BL39, SP6, LU5, LI4
Stagnation of liver qi
a. Manifestations: Urine retention or dribbling urination, hypochondriac and abdominal distention, red tongue with thin and yellowish coating, wiry pulse.
b. Treatment Principle: Relieve Liver qi stagnation and promote urination.
c. Points Selection: REN3, BL28, BL39, LR3, BL18
Urethral obstruction
a. Manifestations: Dribbling or thready urine, or retention of urine, lower abdominal distention and pain, purplish tongue or purplish spots on the tongue, choppy pulse.
b. Treatment Principle: Promote urination by removing stasis and obstruction.
c. Points Selection: REN3, SP6, ST28, KI5, SP10
Insufficiency of spleen qi
a. Manifestations: Straining distention of the lower abdomen, feeling for urination but without urine discharged, or oliguria with hesitant urination; pale tongue with thin coating, thready and weak pulse.
b. Treatment Principle: Ascend clear qi and descend the turbid qi to promote water metabolism.
c. Points Selection: REN3, BL22, SP6, BL20, ST36
Declining of kidney yang
a. Manifestations: Retention of urine or dribbling and difficult urination; pale tongue with whitish coating, deep, weak and thready pulse.
b. Treatment Principle: Warm KI yang and invigorate the KI to promote urination.
c. Points Selection: REN3, DU4, BL23, BL39, KI10
