Continuity of care
Process by which healthcare providers give appropriate, uninterrupted care and facilitate a patients smooth transition between different settings and levels of care.

Community based care
Healthcare provided to people who live within a defined geographic area.
Centered on individual and family healthcare needs (not the health of a community)
HIPAA privacy rule
Assure that health information is protected while allowing the flow of health information needed to provide and promote high-quality healthcare and protect the publics health and well-being.
Chart
Computerized or paper medical record containing medical records, assessments, nursing docs of plan of care, diagnostic and surgery results.
Discharge planning
Planning for continuity of care.
Ensures patient and family needs are met as the patient moves from the acute care setting to care at home.
Guidelines for discharge planning
- Assess and identify healthcare needs - health data, personal data, caregivers, environment, financial and support services. pg 166
- Setting goals with the patient- mutual and realistic
- Teaching - medications, procedures, treatments, diet, referrals, health status
- Meeting eligibility requirements for home care reimbursement
Kardex card
System used to provide patient information in a brief format.
