Common population with Neisseria meningitidis (Meningococcus)
Children and young adults in winter and spring
Primary clue of pathogen N. meningitidis
Petechiae
Treatment for N. meningitidis
Penicillin G 50,000 U/kg q4h
Prophylaxis for N. meningitidis
Rifampin
Population at risk for Streptococcus pneumoniae (Pneumococcus)
Very young (1-4months) and very old Secondary to primary infections involving the ear or paranasal sinuses
Treatment for S. pneumoniae (empiric)
Ceftriaxone 2g q4h plus Vancomycin
Population at risk for Haemophilus influenzae
Children 6 months - 3 years
Often a complication of primary infection of middle ear, paranasal sinuses or lungs)
Treatment for H. influenzae infection
Cefotaxime or ceftriaxone
Prophylaxis for H. influenzae
Rifampin
When should Dexamethasone be given for CNS infections
BEFORE first dose of antibiotics (reduce inflammation/cytokine production in CSF)
Population at risk for Listeria monocytogenes
Neonates, aged, immunosuppressed
Treatment for Listera monocytogenes
Ampicillin +/- Aminoglycoside (3 weeks)
Population at risk for viral meningitis
Children and young adults
Population at risk for Herpes encephalitis
Neonates and sexually active adults
Treatment for viral meningitis and herpes encephalitis
Acyclovir
Vidarabine or foscarnet (if resistance to acyclovir)
