Clopidogrel Flashcards
How is the clopidogrel turned into the active form
CYPP450 (2C19) enzyme turns it into the active form
What is so special about plavix. Hint it’s similar to ticlopidine but wih a notable difference
It’s a prodrug but it has a slow onset
What are some benefits when clopidogrel to ticlopidine
Not as toxic to body and less thrombocytopenia and neutropenia
What surgery is this clopidogrel commonly used for. Hint same as ticlopidine
Coronary stent placement
What is clopidogrel good at preventing when compared to aspirin
It’s good at preventing a second stroke just like ticlopidine. But it’s even better than aspirin for unstable angina
What’s better for preventing ischemia with patients having unstable angina aspirin or clopidogrel
Clopidogrel or plavix
What does clopidogrel have a black box warning
Some patients are poor CYP2C19 metabolizers and can’t turn it into an active drug. Therefore these people will be at risk for stroke and heart attack. There is some resistance to its effects
Give the percentage on the variation of CYP2C19 gene in various ethnicities
Variation is found in 30% whites, 40% blacks and 55% East Asian of CYP2C19 gene
What test can be done to determine the effects of clopidogrel
Genetic test with a therapeutic strategy
