Chronic Kidney Disease Flashcards
What is it
Reduced GFR and/or evidence of kidney damage over a long period of time
How is GFR assessed
24-hour urine collection measuring creatinine clearance
Stage 1 disease
GFR >90 with evidence of kidney damage
Stage 2 disease
GFR 60-90 with evidence of kidney damage
Stage 3 disease
GFR 30-60
Stage 4 disease
GFR 15-30
Stage 5 disease
GFR <15
Causes of chronic kidney disease
Diabetes Hypertension Chronic GN Polycystic kidney disease NSAIDS, ciclosporin, lithium
By what stage of disease do symptoms usually occur
Stage 4
Symptoms
Fatigue Anorexia Itch Sleep disturbance Nocturia Oliguria
what calcium disorder does CKD cause
Secondary hyperparathyroidism
Biochemistry of secondary hyperparathyroidism
Low calcium, High PTH, High Phosphate
Why is there reduced calcium absorption in CKD
Vitamin D is hydroxylated in the kidney- this is impaired in CKD so calcium cannot be absorbed
why is anaemia a complication of CKD
Erythropoetin is produced by the kidneys- loss causes anaemia
Treatment
Lifestyle change (limit alcohol)
ACEi
Statins (decrease CV risk)
Dialysis at end stage
