chronic disease 1 Flashcards
Non-communicable diseases are
long term in nature,
not acquired by transmission between people, but
share many common lifestyle-related risk factors
Communicable (infectious diseases):
: are acquired by
transmission in some way from one person to another
Chronic illness
iss the irreversible presence,
accumulation or latency of disease states or
impairments that involve the total human
environment for supportive care and self-care,
maintenance of function and prevention of further
disability
Chronic condition
efer to a broad range of chronic
and complex health conditions across the spectrum of
illness, including mental illness, trauma, disability and
genetic disorders. These occur across the life cycle
and are generally long term and persistent, and often
lead to a gradual deterioration of health and loss of
independence
Disability:
when a person experiences a level of difficulty in one or more of the following areas.
areas in disability
* Related to body function or structure (impairment) * Challenges in carrying out everyday activity (activity limitation) * Problems in any area of life, not just health related (participation restrictions)
Co-morbidity:
the presence of two diseases
occurring simultaneously in a person (often because
of shared risk factors).
Multimorbidity:
where someone experience three
or more conditions concurrently.
where someone experience three
or more conditions concurrently.
complex causality with multiple factors leading to their onset,
a long development period, for which there may be no symptoms ie.
Chronic Kidney Disease (CKD) •
a prolonged illness, perhaps leading to other health
complications,
create impairment or disability
affect individuals either alone or as comorbidities
While not usually immediately life threatening,
Acute disease Beginning cause duration diagnosis
Usually rapid
Usually one, identifiable
Short
Commonly accurate
chronic disease Beginning cause duration diagnosis
Slow Often uncertain, especially early on Usually for life Sometimes difficult
Chronic
Disease and
disability
Chronic diseases are the leading cause of death worldwide,
equivalent to 71% of all deaths globally
Chronic disease in Australia
•The proportion of persons with a chronic disease increases with age.
•The proportion of people reporting more than one chronic condition also increases with
age.
National Strategic Framework
for Chronic Conditions
Purpose
: A national policy to assist states and territories to prevent
and manage chronic conditions and improve health outcomes.
National Strategic Framework
for Chronic Conditions objectives
Objective One – Focus on prevention
Objective Two – Provide efficient, effective and appropriate care to
support people with chronic conditions and optimise quality of life
Objective Three – Targeting of priority populations
