What do cholinergic blocking drugs do?
blocks or inhibts ACh in the parasympathetic nervous system (rest & digest)
(ANTI CHILL)
-also known as anticholinergics
What do cholinergic blocking drugs do to the eyes?
dilates pupils (mydriasis)
decreased accommodation caused by paralysis of ciliary muscles (cycloplegia)
What do cholinergic blocking drugs do to the GI tract?
relax smooth muscle tone
decreases gastic secretions
decreases motility and peristalsis
What do cholinergic blocking drugs do to the urinary system?
increases contraction of internal sphincter which results in urinary retention
What do cholinergic blocking drugs do to the respiratory?
decreases bronchial secretions/salivation/sweating
dilates bronchial airways
What do cholinergic blockers treat for respiratory diseases?
-exercise induced bronchospasms
-chronic bronchitis
-asthma
-COPD
Anticholinergics for GI treatments
-IBS
-GI hypersecretory states
Administer Dicyclomine
Contraindications for anticholinergic drugs
-angle closure glaucoma (those w/ increased intraocular pressure)
-acute asthma or other respiratory distress
-myasthenia gravis
AE of anticholinergic drugs
-increased HR
-dilated pupils causing blurred vision)
-decreased salivation
-decreased motility causing constipation
-urinary retention
What is an antidote for anticholinergic drugs?
physostigmine
What meds not to mix anticholinergics with?
antihistamines, do not mix w/ any med that causes pt to be slow, sleepy, drowsy
What is atropine used for?
-bradycardia
-is an antidote for anticholinesterase inhibitor toxicity or poisoning
What is given for an atropine overdose?
physostigmine
Oxybutynin (ditropan) indications
anticholinergic drug
-used for an overactive bladder
Contraindications:
urinary/gastric retention
uncontrolled angle closure glaucoma, increased intraocular pressure
Scopolamine indication
anticholinergic
-used to prevent motion sickness
-to prevent postop, postanesthesia N/V, give this med before surgey
-it decreases/drys out secretions
Contraindications:
angle closure glaucoma
adverse effects of scopolamine
drowsiness, dry mouth, blurred vision
Nursing implications for anticholinergic drugs
-assess for allergies, and presence of BPH, and glaucoma
-tell pt that blurred vision will cause problems w/ driving
-may experience sensitivity to light, instruct to wear sunglasses
-dry mouth may occur chew on candy, frequent mouth care
-older adults may be at higher risk of heat stroke
-limit physical exertion
-encourage increased intake of fluid and salt
Teaching for anticholinergic eye drops
-apply pressure to inner canthus to prevent systemic absorption
