When can a copper IUD be inserted as emergency contraception?
Up to 5 days after first UPSI in the menstrual cycle.
Or up to 5 days after the earliest likely date of ovulation (whichever is later).
When would you give oral emergency contraception?
When copper IUD is not appropriate or acceptable.
When is oral emergency contraception unlikely to be effective?
After ovulation.
If there has been more than 5 days after ovulation, and unprotected sex was more than 5 days ago, can any emergency contraception be given?
No.
When would you consider doing a pregnancy test?
If had UPSI this cycle more than 21 days ago.
When would you use UPA-EC?
If the last unprotected sexual intercourse was <96 hours ago and took place <5 days prior to the estimated day of ovulation
When can you start hormonal contraception after taking each type of oral emergency contraception?
Ulipristal acetate (UPA): 5 days. Levonorgestrel (LNG): right away.
In what specific situations would you use UPA-EC over LNG-EC?
If BMI >26 or weight >70kg (or use double dose of LNG)
In what specific situations would you use LNG-EC over UPA-EC?
If woman has recently taken progesterone.
Use a double dose if woman is taking enzyme inducing drugs.
Why do you need to wait 5 days before starting hormonal contraception with UPA-EC?
Progesterone makes it less effective.
Can you give a woman who has had unprotected sex earlier in her cycle emergency contraception after recent unprotected sex and why?
Yes
Because oral EC does not cause abortion or harm to a very early pregnancy.
