What are the 3 most common carcinomas of the cervix and their prevalence? Which one has worst prognosis?
Squamous carcinoma 80
Adenocarcinoma 15
Adenosqaumous and neuroendocrine 5 (worse)
All histologic subtypes are caused by what?
High risk HPV
Histologically what will we see with squamous carcinoma and adenocarcinoma?
With squamous, we will see nets of cancer cells breaking through the basement membrane of high grade CIN.
Adeno will be a proliferation of the glands.
What is stage 0 of cervical cancer?
Carcinoma in situ, CIN 3, High grade
What is stage 1 of cervical carcinoma and all its sub stages?
Stage 1 big picture is confined to the cervix
1a: preclinical, only by microscope
1a1. Stromal invasion no deeper than 3 mm and no wider than 7mm
1a2: stromal invasion 3-5 mm and no wider than 7 mm
1b: still only in the cervix, but beyond 1a2.
What is going on during stage 2 of cervical carcinoma?
Beyond the cervix but not to the pelvic wall. It has invaded the vagina, but not the lower 1/3.
What is stage 3 of cervical carcinoma?
To the pelvic wall and lower 1/3 of vagina
What is stage 4 of cervical carcinoma?
Beyond the true pelvis and into bladder and rectum. Also included all anything that has metastasized
Which 2 LNs are invaded first?
Pelvic and then para aortic
Distant metastasis of cervical carcinoma includes what 3 places?
Liver, lungs and bone marrow
What is usually the cause of death for patients with cervical carcinoma?
Local problems, not from metastasis
What is the ideal age we want boys and girls HPV vaccinated?
By 11 or 12.
And if not, then women by 26 and young men by 21
So what is the problem with pediculosis pubis?
Lice in the pubes. Sexually transmitted disease. Can get up to the eye and cause pediculosis ciliaris
