(NSTDM) Name. Strength (where appropriate). Total amount to dispense OR total period of supply Dose and frequency of dose If for internal administration (i.e. by injection, insertion into any body cavity, or swallowing) Method of administration if for external application
For which groups of people, the prescription item fees or (co-payment) is waived by govt?
Children under the age of 13

(Adult) Age 18 years & over
Patient Category Code J?
Junior - age 14 - 17 years
Patient Category Code X ?
Exempt after 20 Rx - Families and individuals collected 20 new prescription items in a year do not have to pay any more prescription co-payments for the rest of that year.

High use health card (HUHC) holder.
