CBT sz process

A

1-1 sessions with patient and therapist, weekly 8-20 sessions
Help patient cope and control behavior rather than eliminate symptoms
Process:

-Identify- irrational or upsetting thoughts, logging them as homework
-Challenge – look for evidence for or against delusions and beliefs to see they do not match reality, helping to change them and reduce their stress
-Normalizing- talk about life before symptoms to make sz seem normal, easier to cope with
-Focusing and reattribution- rationalize voice of hallucination to own inner voice
-Belief modification- challenging thought, no evidence for them to act on delusion
-Change –coping strategy to deal with upsetting thought, may be asked to change behavior and record experiences

Found that 10/12 patients showed a decrease in their belief in their delusions.5/12 managed to reject their delusions at the end of their therapy measured via self-report
Is effective in reducing symptoms such as delusions
OTOH only works for some patients as not everyone benefited-may only be effective in some cases?

Assessed CBT in a case study with a female sz patient called carol and found considering improvement in functioning and symptoms over a 3-year treatment period and 1 year follow up
Symptoms are reduced, overall functioning is improved

A

Sensky 2000

had some patients undergoing CBT from Newcastle, county Durham and London compared to a control group doing ‘unspecific befriending’. Both interventions resulted in similar reduction in positive and negative symptoms
Do not know the improvement is actually because of the therapy or just a correlation
