why is imipenem administered in combination with cilastatin?
imipenem is inactivated in the kidney by enzymatic activity
so it is given with cilastatin with is a specific enzyme inhibitor which blocks its renal metabolism
which cabapenem must be given in combination with cilastatin because it is partially inactivated in the kidney (cilastatin is a specific enzyme inhibitor which blocks its renal metabolism)
imipenem
which carbamapen has less seizure inducing potential and therefore can be used to treat CNS infection

CNS disorders - risk of seziures
epilepsy
elderly (ertapenem)
3 interactions of imipenem
- ganciclovir & valganciclovir - these are predicted to increase risk of seizures when given with imipenem. AVOID
- imipenem decreases conc of valproate - avoid
ertapenem and meropenem 1 interaction
valproate - these abx decrease the conc of valproate so avoid
MOA of carbapenems

inhibits cross linking of peptidoglycan during cell wall synthesis
bactericidal - kills the bacterua
